Patient Information

Cosmetic Procedures Policy

Dr. Judge and the team at Central Florida Dermatology Associates will always develop a customized treatment plan to bring your skin to its optimal level of health. In order to accommodate for our patient’s needs, we ask that half of the procedure costs are paid at the time of scheduling, and the remainder by the day of service or prior to the day of treatment. Missed appointments or cancellations requested less than two business days prior to the schedule time will result in forfeiture of the procedure cost.

If you have any questions prior to your appointment please do not hesitate to contact us so we can address your concerns as soon as possible.

No Show Policy

If you do not call us within 48 hours of your appointment to cancel, you will be charged a $25 cancellation fee. If you call us within 24 hours prior to the appointment and reschedule, the cancellation fee will be waived.

Insurance Card Policy

As a courtesy to you, our practice will file all insurance claims. However it is your responsibility to supply our office with the current and correct insurance cards at the time of service for each visit. If you come without the insurance card, full payment is expected at the time of service.

Central Florida Dermatology Associates requires payment or co-payment at the time of services rendered. We accept:

Appointments

To schedule an appointment for a consultation or service with Dr. Judge or any of our staff, please call Central Florida Dermatology Associates at (407) 481-2620 or e-mail us at appointments@centralfloridaderm.com with:

• Reason for Your Visit

Please arrive at least 15 minutes prior to your appointment so that you have enough time to fill out your paperwork. If you arrive  after your scheduled appointment you may be asked to reschedule.

Minor's Policy - Established Patients Only

A minor that is 16 or 17 years of age can be seen by him/herself with a written consent and ID from parent/legal guardian.

A minor under the age of 16 can be seen with another legal adult with a written consent and ID from parent/legal guardian.

The providers have right to refuse treatment at any time should it be a case which the presence of the parent/legal guardian is required for treatment.

Medical Records

To maintain confidentiality of our patients, it is our policy not to release any information regarding medical or financial history with anyone without authorization. If you need to request medical records, please complete the following interactive form online:

If you have any questions about obtaining medical records from Central Florida Dermatology Associates, please call (407) 481-2620 extension 1511. Our office will contact you within 48 business hours and process the request as soon as we receive your signed authorization form.

Prescription Refills

If you want to have your prescription refilled, you must have been seen by Dr. Kathleen Judge or one of the Physician Assistants within the past 12 months. You can call us at 407-481-2620 ext 1504.

If we receive your request for a prescription refill and you have not been seen within the past 12 months we will contact you to schedule an appointment.
