Please arrive at least 15 minutes prior to your appointment so that you have enough time to fill out your paperwork. If you arrive after your scheduled appointment time, we will try to accommodate you but there are occasions were you may be asked to reschedule.
We understand that things happen, but it does affect other patients when an appointment is missed. Rather than charge a missed appointment fee, we allow for 3 missed appointments without a call. After the third missed appointment without a call we reserve the right to refuse service. We hope you understand that this is for the benefit of our patients who would like to be seen as soon as possible.
Dr. Judge and the team at Central Florida Dermatology Associates will always develop a customized treatment plan to bring your skin to its optimal level of health. In order to accommodate all of our patient’s needs, we ask that half of the procedure costs are paid at the time of scheduling, and the remainder by the day of service or prior to the day of treatment. Missed appointments or cancellations requested less than two business days prior to the scheduled time will result in forfeiture of the procedure cost.

If your insurance company charges a co-pay for your office visit, the co-pay may not cover the procedures performed while in the office. Depending on your plan, procedures such as biopsies, surgeries and wart treatments may be subject to your policy’s deductible.
Due to the uncertainty in the insurance marketplace, it is your responsibility to ensure that we are listed as a provider in your network and your visit will be covered by your specific plan. We will do our best to assist you.

If you want to have your prescription refilled, you must have been seen by Dr. Kathleen Judge or one of the Physician Assistants within the past 12 months. You can call us at (407) 481-2620.
All minors must be accompanied by a legal guardian during their first visit. The legal guardian may be asked to prove legal guardianship, so please arrive prepared.
A minor that is 16 or 17 years of age can be seen by him/herself with a yearly office consent, yearly minor consent, and a Photo ID from parent/legal guardian.
A minor under the age of 16 can be seen with another legal adult with a yearly office consent, yearly minor consent under 16, and a Photo ID from parent/legal guardian.
The providers have the right to refuse treatment at any time should there be a case in which the presence of the parent/legal guardian is required.

To maintain confidentiality of our patients, it is our policy not to release any information regarding medical or financial history to anyone without authorization.
